Here are some questions that we hear a lot from parents.

	Centre Policy This document must be signed and submitted to the Centre at the time of registration.
	PAD Authorization Form This form must be signed and submitted at the time of registration.
	Student Vacation Form Please fill out this form if you are taking a whole month of vacation without work and do not wish to be charged tuition.
	Termination Form Please fill out this form and submit it to the Centre if you plan to discontinue your child's study at Kumon. Remember that you must give a full month's calendar notice (see Centre Policy for details).
